F.O.P.® Active Member Re-Instatement
PLEASE NOTE: THIS FORM IS FOR ACTIVE (LAW ENFORCEMENT) MEMBER USE ONLY. DO NOT USE THIS FORM IF YOU ARE AN ASSOCIATE MEMBER
Lodge Dues are payable on an Annual basis effective September 1st of each year, and are payable within Thirty (30) days of that date (September 30th)
This online form is provided as a convenience to suspended Actvie Members who wish to submit a reinstatement back into Good Standing with the Lodge. In submitting this Re-instatement Request:
- You hereby request that FOP® New Jersey Lodge No. 46, Inc. re-instate your Active Membership back into Good Standing
- You attest that you are still eligible to be an ACTIVE member of this Lodge.
- That you will continue to abide by all the laws and rules of this Lodge and of this order.
- That you understand that you will maintain your Membership in Good standing from this point forward, and will submit all future membership renewals no later than Thirty (30) Days from the date due (September 1st) each year, and
- That you understand that by failing to submit future renewals within the mandatory time frame, that your membership in the Lodge and in the Order may be subject to permanent revocation.
